What to Report:
Members are encouraged to report the following incidents or concerns:
- Injuries: Any injury, no matter how minor, including cuts, burns, or other accidents requiring first aid or medical attention.
- Tool or Equipment Malfunctions: Issues such as damaged, malfunctioning, or improperly functioning tools or equipment.
- Unsafe Conditions: Hazards such as spills, obstructions, faulty wiring, or poorly maintained workstations.
- Near Misses: Situations where an accident was narrowly avoided.
- Policy Violations: Behavior that disregards the Code of Conduct, safety guidelines, or facility use policies.
How to Report:
Follow these steps to report an incident or concern:
- Seek Immediate Assistance:
- In the event of an emergency (e.g., severe injury, fire), dial 911 immediately.
- Notify other members nearby and ensure the area is safe to approach.
- Notify a Board Member:
- For non-emergencies, report the incident to a board member as soon as possible. You can find board member contact information posted in the facility or on our Discord server.
- Document the Incident:
- Provide a clear and detailed account of what happened, including the time, location, and nature of the incident.
- If possible, include photos or videos to document unsafe conditions or damage.
- Submit an Incident Report:

- All reported incidents will be reviewed by a board member or a designated safety officer.
- Necessary actions, such as repairs, policy updates, or additional training, will be taken to prevent similar incidents.
